What it is
Compare the program on paper to the work on the floor.
A gap assessment is not a surprise inspection and it is not a binder rewrite on day one. It is a structured look at written programs, training records, permits, inspections, and the jobs people actually do. The output is a ranked list: missing, out of date, written but not followed.

Scope
One day on a small site. More days when the operation is larger.
Length follows the size of the site, how many processes are in play, and how complete the records are. Two common shapes:

One-day assessment
A focused walk of one plant, yard, or jobsite. Opening with whoever owns the program. Floor and records in the same day. Close-out with a short verbal debrief so the site is not waiting a week to hear the obvious gaps. Written findings follow.
Fits a single location, a limited process set, or a manager who needs a ranked punch list before an upcoming visit.

Multi-day assessment
More than one area, shift, or process. Time on the floor plus time in the records: programs, training, inspections, incident files, contractor control. Interviews with supervisors and the people doing the work.
Fits a larger plant, a multi-crew jobsite, or a site that has not had an outside look in years. The write-up is longer because the operation is.
